Applying for a trade account:
We have tried to make our trade account application process as easy as possible. Simply follow the steps below to set up an account:
Trade application form
- Download our trade application form using the link above.
- Save a copy to your computer.
- Fill in the details and email the completed copy to firstname.lastname@example.org.
- If your application is accepted, you will receive an email with details of your credit limit and will be granted 30 day credit terms.
- We will send you a Healthspan Elite trade account order form. For your first order and all future orders, simply fill in the quantity of products you require on the form and send it to email@example.com

Terms and conditions
All team trade accounts are subject to the standard Healthspan Elite® terms and conditions which can be found here.
In addition the following terms also apply:
- Payment of all invoices will be 30 days from the invoice date.
- Team trade orders should be detailed on the order form provided and sent to email@example.com. A Purchase Order Number should be specified where required by the Payment Team of the ordering company.
- Payment of all invoices should be made by Bank Transfer direct to our account. Details will be provided once your application has been approved.
